What is Laser Liposuction?

Liposuction simply involves suctioning fat from the body. Back in the day, it was performed only in the operating room under general anesthesia. Very large suction cannulas were used and results were mixed. In the 80s, liposuction under local anesthesia was developed by a Dermatologist! It is now the preferred method of liposuction using very small microcannulas. It delivers much better results with little to no downtime. Around 2006, Smartlipo Laser Liposuction was invented. This involved the insertion of a small laser fiber through the same incisions. The heat generated by the laser caused skin tightening, melted fat, and cauterized blood vessels. We now have many different types of laser liposuction devices available.

Is the goal of liposuction to remove as much fat as possible?

No, it’s a lot more complicated than that. In fact, it’s not the fat that is removed that is important. It’s the fat that is left and what condition it is left in. Not only should the fat layer be smaller but it needs to be smooth and well contoured. This is where the art of liposuction trumps the science of liposuction. It takes artistic skill to sculpt and contour the body areas so that they look natural and improved. Think of the analogy of sculpting a statue from a block of stone. If we just took as much stone as possible with a sledgehammer, without paying attention to the shape, the resulting statue would not look good. But if we take our time, use fine instruments and focus on the artistry of what we are doing, we end up with a beautiful sculpture!

What happens after you remove the fat with liposuction?

Space is created between the skin and muscle where the fat was removed. Your body will naturally fill this area with fluid which causes postoperative swelling. Your body will remove this swelling over the next several weeks and months and is helped by wearing compression garments, massage, and exercise. During the healing process, the body will reorganize the remaining fat and contract the skin and connective tissue. We can see more than 50% skin tightening after aggressive power-assisted liposuction. Final results will take 3-6 months.

What do you do with the fat from liposuction?

We usually dispose of the fat. However, we now know that fat is rich in stem cells and can be transferred throughout the body for volume replacement. Fat transfer works great for buttocks augmentation, natural breast augmentation, and facial volume replacement. As it’s your own fat, it is much safer than implants or injectables and the stem cells can provide long-lasting results and rejuvenation.

How is Laser Liposuction used? (lasers only used by Surgeon’s choice)

Liposuction using a cannula is very effective to remove fat from anywhere on the body. However, it is not just about removing fat. It is the condition of the fat layer that we leave that is more important. This is very difficult to achieve, especially in areas with small amounts of fat. Being too aggressive can leave unsightly grooves and dents. This is where laser liposuction comes in. The heat generated by laser-assisted liposuction helps to melt fat cells. This fat can then be suctioned or allowed to drain out. So the laser is perfect to melt fat in smaller areas and smooth out the fat in larger areas. The heat generated by the laser also stimulates some immediate skin tightening and induces new collagen formation. As a result, the overlying skin contracts to the new, smaller fat layer giving the perfect liposuction results we want!

How do I choose a laser liposuction procedure?

Smartlipo laser liposuction by Cynosure was the original laser on the market. There are many competitors now like Slim Lipo, Cool Lipo, Pro Lipo, etc. The only differences between these lasers involve the wavelength of light used and power. Smartlipo Triplex uses 1064, 1440, and 1320 wavelengths that have research to back their clinical effectiveness. However, there are no studies to show the advantage of one laser over another. Remember the effects of laser liposuction are due to heat delivery. All of these lasers deliver heat to the fat and skin. It is the consensus among the liposuction community, that the laser is unimportant compared to the amount of heat delivered. So it is the surgeon who is performing the procedure and his experience with laser-assisted liposuction that will give you the best results.

What should I look for when choosing Laser-assisted Liposuction?

The main things to require for your liposuction procedure include tumescent anesthesia, microcannula liposuction, and aggressive liposuction. These three components will give you the lowest risk of complications, the best results, and the fastest recovery. Ask your surgeon specifically about each of these techniques and make sure he will be utilizing them. Skipping any of them can affect your results and risk complications.

Is Laser-Assisted Liposuction safe?

The effectiveness of the laser depends on heat delivery. With the use of heat, there is the risk of burns. Many of the lasers now have temperature controls but this doesn’t replace the experience of the surgeon to prevent such complications. Check your surgeon’s reviews, pictures, etc. However, the incidence of burns from laser liposuction is very low and rare.

How fast will I see results from laser-assisted liposuction?

The heat generated from laser-assisted liposuction will cause some immediate skin tightening. However, with the swelling and loss of fat, it will be hard to see for several days. As the swelling subsides and the skin tightens, you will begin seeing the smooth, smaller fat layer and tighter skin. This is the fun part as you see more and more results every day! It usually takes 3-4 weeks to begin to see where you will settle down and 3-6 months for final results.

Vaser Liposuction

Vaser liposuction uses a probe that generates ultrasonic sound waves. These ultrasonic waves make tiny bubbles that disrupt the fat cells. This makes it easier to remove more fat. Vaser is ideal in areas that have more fibrous fat like the back and love handles. Vaser is also ideal for body contouring and high-definition liposuction.

What do I need, Vaser, Laser, or Traditional Liposuction?

The choice of the specific instruments used during the liposuction procedure belongs to the surgeon. It is not necessary to use every instrument, every time. The surgeon using their training and experience will select the procedure to give you the best possible results.

Liposuction vs Tummy Tuck

Deciding between liposuction alone or liposuction with a tummy tuck can be a difficult one. Depending on the condition of the skin, liposuction alone can achieve more than 50% skin tightening. This may be enough for many patients. If the skin has poor elasticity or many stretch marks, a tummy tuck may be necessary to remove the excess skin. The tummy tuck also allows for tightening of stretched-out abdominal muscles. On the downside, a tummy tuck leaves a large scar and has a longer recovery. Liposuction Risks and Common Side Effects

Once the liposuction surgery is over, the recovery and healing process is just beginning. The tumescent anesthesia that is left in the fat layer is allowed to drain. Any fluid that doesn’t drain, will be safely absorbed by the body. Space where the fat once was, is now partially empty. A fluid that circulates in the body between cells, lymph, begins to accumulate in this area. This causes the most common liposuction side effect, swelling (lymphedema). The patient will wear a compression garment to squeeze this fluid out of the area until the body heals the lymphatic channels. Massage and physical activity also assist the resolution of liposuction swelling. The swelling begins to lessen anywhere from 3-6 weeks after the procedure but sometimes takes several months to completely resolve.

Blood loss is limited during the procedure due to the epinephrine in the tumescent anesthesia. However, as time passes, the epinephrine goes away and some bleeding or bruising is possible. This is controlled by keeping the compression garment on and avoiding strenuous activity. Patients susceptible to bruising can be helped by the supplement arnica and vitamin K.

During the liposuction, some peripheral sensory nerves can be damaged. This can result in some transient numbness and tingling sensations. These nerves will repair themselves with time. Some other common liposuction side effects include scars and loose skin. We do have to make small incisions and we try to place them in inconspicuous spots. They can leave small scars that can be treated with lasers and topical products. Most of the time after liposuction, the skin will tighten with time. The heat from laser liposuction like Smartlipo will further improve skin contraction. Once in a while, additional procedures may be necessary to deal with the loose skin after liposuction.

Rare Liposuction Side Effects

Some of the less common Liposuction Side Effects can include uneven results and grooves/dents, skin color changes, and burns. Liposuction is more of an artistic pursuit than a scientific one. The surgeon is human and can make errors and cannot control how the patient’s body reacts to the liposuction. Once the swelling resolves, uneven results or liposuction defects may be noted. The good thing is that they can easily be corrected with a touch-up procedure. In the same vein, there can be skin color changes after the procedure. Most of these resolve with time. Burns are very rare and can be avoided by an experienced surgeon.
